Frequently Asked Questions

  • When you call in to make an appointment our staff will ask you for some basic demographic information (contact information, date of birth, and what office location you will be seen at). They will also ask about your area(s) of concern and if you have a counselor in mind to work with. If you are open to seeing any counselor, they will try to pair you with the counselor that best fits your area(s) of concern and scheduling needs.

    Once you decide to meet with a particular counselor our staff will offer some appointment times to you. Once an appointment time has been set, you will receive a client portal invite via email. Please log in to the portal and complete the intake forms before your first session. If you do not have the forms completed before the first session, the first 10 minutes of your appointment time will be spent completing them on site. Our providers cannot meet with you until these forms have been completed.

  • We accept Blue Cross Blue Shield (any state), Cigna, HealthPartners, Aetna, UnitedHealthcare/UMR/Optum Behavioral Health, Midlands Choice, Iowa Medicaid, Iowa Total Care, Wellpoint, and Molina insurance. Private pay is also available if you do not have insurance or have a policy that we do not accept. Please call your insurance to verify coverage before the first session. We can make no guarantees of coverage as your plan may have a deductible to meet before coverage kicks in.   • Per our informed consent statement, any cancellation with less than 24 hours notice is considered a “late cancellation”. In the absence of illness or an emergency on the day of the appointment your counselor may choose to charge a $75 late cancellation/missed appointment fee. To cancel your appointment please call 319-800-5564 and leave a voicemail if your call is not immediately answered. You may also reach out to your counselor directly via email.

  • Our goal is to find the best therapeutic match for you and your counselor. If you feel as though you are not getting as much as you would like to from sessions with your counselor and you feel that there may be a better match for you within the practice, we are happy to set you up with another clinician. Please reach out to us via phone or email to initiate that process.

Printable Intake Forms

As part of the scheduling process all clients are sent a client portal email invite to log in and register as a client. From this portal account they can complete the intake forms, see upcoming appointments, and access telehealth sessions. To account for any technological issues/barriers to completing the intake forms via the client portal, we also have printable versions available for download below. Feel free to print and fill the following forms out and bring them to your first session. Your counselor will collect them from you.

For divorced/seperated parents of minors (under 18 years old): Please have each custodial parent/guardian complete an informed consent form to allow for mental health treatment of the minor. Your counselor may decline to offer services if both custodial signatures are not received before the session.
